FedBucks is a federation of 45 GP practices covering a population of over 485,000 patients across Buckinghamshire. We began in 2016 and now employ around 200 members of staff across our head office sites, and our planned and unplanned care services.

Aylesbury Central Primary Care Network (PCN) is a collaboration of two practices in central Aylesbury, Whitehill Surgery and Berrycroft Community Health Centre, collectively we care for a patient population of 41,000. The successful applicant will be working with a team of clinical pharmacists and become part of a fast-moving PCN looking for innovative ways to meet our patient needs.

Job Summary

Primary Duties

This is an exciting opportunity for the ‘right’ person who wants to work supporting people by coordinating their health and social care services. We are seeking a dynamic individual who embraces change and is forth coming with innovative ideas to improve patient care. The care coordinator will be involved in supporting the clinical team to proactively identify and work with patients, including the frail/elderly and those with long-term conditions, to provide proactive, person-centred care planning, helping coordinate care, by bringing together the different specialists whose help that individual might need. This might involve a wide range of services, such as hospital care, community care, social care, housing and the voluntary sector.

 

This new role has been developed to support the delivery of better outcomes for patients living with multiple long-term conditions, to help them improve the quality of their life, fostering self-care, independence and patient choice. Supporting them to understand and manage their conditions and ensuring their changing needs are addressed.

 

In line with support provided to other staff groups, there will be networking and training opportunities across the network, to support the development of the care co-ordinator role within the network, and wider primary care in Buckinghamshire.

 

Responsibilities

  • Provide operational support within the care co-ordinator workstreams (listed below)
  • To act in the patients’ best interests, working to balance the complex demands of the service
  • To conduct patient and staff surveys
  • Provide administrative support for individual workstreams and projects for the PCN.
  • The post holder will be able to effectively prioritise their workload and have experience of exercising initiative and decision making without direct supervision.
  • Produce documents, briefing papers, reports and presentations using a range of office software.
  • To undertake specific assigned tasks, project support, or development / change work which may arise from time to time.
  • To provide support to the PCN senior management team in the running of key systems and procedures.
  • The post-holder will need to become familiar with all national and local quality standards for primary care.
  • The post holder will need to be familiar with the Direct Enhanced Services and key frameworks within this.
  • The successful candidate will undertake general administrative duties

 

Person Specification

Education / Qualification / Experience

Qualifications

  • GSCE or above in Maths and English
  • Higher level qualification such as NVQ in adult care Level 3
  • Qualification relevant to health or social care

Experience

  • Experience of working in health, social care and other support roles in direct contact with people, families or carers
  • Experience of working in a multi-professional team environment
  • Confident in using Email and sound IT knowledge
  • Good written English and excellent record keeping skills
  • Experience of working in primary care
  • Experience of working as a care coordinator
  • Experience or training in personalised care and support planning
  • Experience of working with elderly or vulnerable people, complying with best practice and relevant legislation
  • Proficient in use of EMIS
  • Experience of working with reports in EMIS

Skills & Knowledge

  • Understanding of and commitment to equality, diversity and inclusion
  • Knowledge of policies and procedures, including confidentiality, safeguarding, long working, information governance and health and safety
  • Knowledge of how the wider NHS works, including PCNs
  • Knowledge of long term conditions and the complexities involved – medical, physical, emotional, and social
