Vision Statement
Aylesbury Central PCN believes good patient care should focus on holistic support delivered by a happy team in a supportive, learning environment. We strive to help our local population live full lives and make a real difference to the health inequalities we see every day by tackling the multiple underlying causes of disease. We have a combined list size of just over 40,000 patients. Aylesbury Central PCN is led by Dr Rhiannon Turner, ACD and supported by Racheal Alley PCN Manager. Each practice has a nominated PCN Clinical and Managerial representative.

''I just want to say a huge thank you to the social prescribing team who called me after a home visit from the GP. I have had to try and organise a carer for my In-Laws urgently and did not have a clue how difficult it would be. Suzi chatted through the types of care and the referral options in adult social care and guided me through the options. I received a prompt email providing links to PA providers. Great support from a knowledgeable team. Thank you.''
