Preventing Diabetes at Aylesbury Central PCN
A 41 yr old man was referred into the Health and Wellbeing Service who was struggling to lose weight. After several ‘failed’ attempts and the impact this was having on his health, it was important he began making lifestyle changes. During the initial consultation this patient had the time to explain and discuss previous weight loss attempts as well as his current diet and lifestyle.
From his own research had been trying to make changes in a way that wasn’t working for him, in this case had replaced fat with carbohydrates. The Health and Wellbeing Coach played an integral part in educating and offering accountability in order for him to begin making lifestyle changes to achieve his goals of weight loss.
Small realistic goals were set throughout the health coaching sessions which were identified by the patient, these could be adapted, progressed or regressed at which ever point the patient felt necessary. These were set out to begin focusing on the types of food he was eating, opting for a low carb, real food approach. From using this technique this patient began living a healthier lifestyle, in turn improving his health significantly. From losing an amazing 54kg, this patient is no longer a diabetic, no longer has sleep apnoea and has his blood pressure back within a normal range.
