AYLESBURY CENTRAL PCN UPDATE
Liver Cancer Case Finding - New Funded ProjectWe are delighted to confirm Aylesbury Central PCN has been selected as just one of seven PCNs nationally to deliver a Liver Cancer Case Finding project. This project aims to increase early detection of liver cancer to help achieve the Long Term Plan ambition for 75% of all cancers to be diagnosed at an early stage (1&2) by 2028.
We, as a PCN intend to operate a community-based project aiming to provide early detections with patients at high risk of HCC (the most common form of Liver Cancer) working closely with secondary care and neighbouring PCNs (primary care networks). The aim of the project is to compliment the services already being offered and provide care and on-going support within the local community.
We plan to work collaboratively with Health Share, Buckinghamshire Healthcare Trust and One Recovery Bucks. Health Share, who support our project, will be able to provide community based ultrasounds and fibroscans. There are significant wait times for scanning both ultrasounds and fibroscans within Buckinghamshire Healthcare Trust, by working in partnership with Health Share, patients will be able to be seen, assessed, and scanned with little to no wait time thus significantly reducing the time from identification to diagnosis and management. We will also engage with One Recovery Bucks to support patients with alcohol dependency.
The key milestones during this project will be:
- Increase the population with confirmed cirrhosis, with an active management plan
- Increase in numbers of patients with identified liver disease engaging with lifestyle changes
- Establish an annual recall system for patients with liver disease
