FedBucks is a federation of 45 GP practices covering a population of over 485,000 patients across Buckinghamshire. We began in 2016 and now employ around 200 members of staff across our head office sites, and our planned and unplanned care services.

Aylesbury Central Primary Care Network (PCN) is a collaboration of two practices in central Aylesbury, Whitehill Surgery and Berrycroft Community Health Centre, collectively we care for a patient population of 41,000. The successful applicant will be working with a team of clinical pharmacists and become part of a fast-moving PCN looking for innovative ways to meet our patient needs.
Job Summary
Primary Duties
Primary Duties & areas of Responsibility:
Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement their personalised health and care plan.
Provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare; empowering them to take more control in managing their own health and wellbeing, to live independently and improve their health outcomes through:
1) Providing interventions
2) Supporting people to establish and attain goals set by the person based on what is important to them.
3) Working with the social prescribing service to connect them to community-based activities.
4) Working with the Long-Term Conditions (LTC) team to deliver specific health advice, where appropriate.
5) Working to increase the prevalence of treatable LTCs by undertaking health checks.
6) Inputting data including appropriate disease and referral coding
7) Provide support to local community groups and work with other health, social care and voluntary sector providers to support the patients; health and well-being holistically.
8) Help to establish and co-ordinate patient groups to aid self-management, peer support, and the creation of patient communities
11) Support colleagues to improve their skills and understanding of personalised care, behavioural approaches.
12) Undertake all the necessary training and induction for the role
13) Raise awareness within the PCN of shared decision making and decision support tools and supporting people in shared decision-making conversations.
15) Explore and support access to a personal health budget, where appropriate, for their care and support
16) Utilise existing IT and MDT channels to screen patients, with an aim to identify those that would benefit from health coaching
17) Work with the PCN and practices towards a number of different outcome and quality improvement measures including QOF, QIPP and IIF
Responsibilities
The successful candidate will play a crucial role In the development of the health coaching services. Providing support through 1-1 patient led consultations, delivering group consultations to specific cohorts and bespoke exercise classes where there is a patients need. Supporting with ongoing projects: prehab project and Liver study will also be factored into current workstreams.

Person Specification
Education / Qualification / Experience
- NVQ Level 3 Health Trainer qualification or other relevant professional academic qualification
- Educated to degree level or related discipline
- Training in motivational interviewing/coaching
- Training in delivering psychological interventions
- Experience of project management in a healthcare or voluntary setting
Skills & Knowledge
- Understanding of the importance and process of helping people with long – term conditions to develop their knowledge, skills and confidence in managing their health and the range of models and tools available
- Â Understanding how to apply health coaching in independent/group settings
- Able to work within a biopsychosocial model, using a range of tools and techniques to enable and support people, such as agenda setting, goal setting, problem solving
- Demonstrable skills in supporting behaviour change
- Excellent group and one -to -one facilitation skills including conflict resolution
- Skilled in active and reflective listening, building trust and rapport quickly
- Good people management skills
- Ability to work with minimal supervision and act decisively and ask for help when needed
- Excellent communication and presentation skills
- Ability to work with a range of clinical and non-clinical personnel as part of a team
- Ability to work independently and effectively with a high degree of motivation
- Ability to prioritise and work to deadlines
- Ability to define, collate, analyse and interpret data
- Able to utilise databases and information technology, including word processing, spreadsheets and presentation packages effectively
